High revs at Gear 5

Hello guys, firstly hope everyone is having a wonderful cny! Got a question to ask about :doh: recently when drriving my SiR4, at gear 4 and 5 the rev and speed dont match. When I step on the gas harder the revs continue climbing up to 4k5k but my speed isnt climbing together :cry: anyone know whats wrong? Isit the clutch worn off alrdy or something else? Thanks alot! :D

Im not too sure but I know that when I go hard on the gas pedal and the rpm hits 5k I do smell something funny
